CIMS Bands Enrollment Packet

  • PARENTS/GUARDIANS – Complete THIS FORM. Be sure to fill out every section until it tells you that you have completed the packet.
    • Parents/Guardians will be required to sign in to your Google account to complete this. In order for your signature to be accepted in this format, it must be connected to your own Google account. (Students, this means you CANNOT fill out this form for your parents.)
  • PLEASE NOTE – Under the Instrumental Rental Contract section, it is important that EVERYONE agrees to the terms of the loan agreement, even if you own your own instrument. If your personal instrument has to go into the shop for repairs, we cannot loan you a school instrument if you do not select I DO AGREE.
